Understanding Surrogate Compensation at CFC

Surrogate “Pay” vs. Surrogate Compensation: What’s the Difference?

Many people search for “surrogate salary” or “surrogate income,” but surrogacy compensation isn’t structured like employment.

Instead of hourly wages or a standard paycheck, compensation is:

  • Outlined in a surrogacy legal contract
  • Paid in structured installments throughout the journey
  • Paired with reimbursements so you’re not paying out-of-pocket for surrogacy-related expenses

In other words, it’s a compensation package designed to recognize your time, commitment, any physical discomfort, the responsibilities you are undertaking, and the medical and legal complexity of the process.

What Is CFC’s Surrogate Base Compensation?

CFC’s current surrogate base compensation* is:

  • $60,000 for first-time surrogates 
  • District of Columbia (DC) and Maryland surrogates will receive $65,000

*For GC applications submitted after January 1, 2026.

We currently match gestational surrogates who live in the United States except “red light” states as classified on our Creative Family Connections US Surrogacy Law Map™.

What Does Surrogate Compensation Typically Include?

While surrogacy is a rewarding and incredible gift, it is also a complex medical and legal process that requires significant time and energy along the way.  Intended parents provide three types of surrogacy pay in exchange for your dedication, time, and sacrifice.

At CFC, compensation is generally made up of:

  • Base compensation (your primary compensation)
  • Additional stipends and allowances (for specific steps or needs during the journey)
  • Reimbursements for surrogacy-related expenses (so you’re not paying out-of-pocket)

As a surrogate, intended parents will cover your expenses, from your medical procedures to screening costs and your legal fees. 

Surrogacy-Related Expenses: What’s Covered?

Surrogacy comes with real-life logistics, including appointments, travel, and time away from work. Your contract will outline what intended parents will cover.

Common categories include:

  • Medical and screening-related costs
  • Frais de justice
  • Travel-related expenses for medical screening and embryo transfer
  • Meal stipends while traveling for medical screening and embryo transfer
  • Maternity clothing allowance
  • Transfer stipends

You may also receive compensation that covers lost income or limited paid time off you and your spouse incur from missing work due to surrogacy appointments.

How Do Monthly Allowances Work?

In addition to base compensation, all CFC surrogates receive a monthly allowance during pregnancy. This monthly payment helps support the everyday costs that can come with pregnancy and frequent appointments.

Repeat Surrogacy Journeys: What Changes?

If you’re a repeat surrogate, you already know the ropes and you’ve proven your ability and dedication to carry a gestational pregnancy to term safely. Because of that, repeat surrogates are often in high demand.

At CFC:

  • A repeat surrogate will generally receive $5,000 more than CFC’s current surrogate base pay.
  • For example, a repeat surrogate would receive $65,000 as base compensation (which is $5,000 more than our current base pay of $60,000).

Real-Life Examples (What Total Compensation Can Look Like)

Every journey is different. Medical protocols, transfer attempts, pregnancy outcomes, and personal circumstances all impact what your total compensation looks like.

Here are a few real-life examples from CFC to show how base compensation plus stipends can add up:

  • Carrie (Alaska): Approximate total surrogate pay received $83,500+
  • Ashley (Arizona): Approximate total surrogate pay received $67,950+
  • Haley (Maryland, second-time CFC surrogate): Approximate total surrogate pay received $76,450+

These examples include items like transfer stipends, monthly allowance timing, maternity clothing, potential procedure-related stipends, and (in some cases) pumping breastmilk post delivery.

 

A Quick Note on “Too Good to Be True” Numbers

When searching for information about how surrogates are paid, you’ll find that some surrogacy agencies advertise headline numbers that can be confusing without the full breakdown.

A good rule of thumb is, if it seems too good to be true, it probably is. The best way to compare agencies is to ask for a clear, written outline of what’s included in base compensation, what’s paid as stipends, and what’s handled as reimbursements.

Also, a note of caution: If an agency proposes to give you a big lump sum and let you decide how to allocate the money, stop and think. What if you have a lot of medical complications in this journey? If that’s the case, you could have more medical bills, more lost wages, and more expenses with a total exceeding that hefty sum the agency proposed to give you. What’s really happening is the agency has shifted the risk from the intended parents to you. If your surrogacy journey is low risk, all is fine; but if your surrogacy becomes high risk, you will need to reach into your own pockets to cover surrogacy-related expenses. And that is not fair to you.

Do surrogates earn extra if they’re carrying twins or triplets?

Yes. In the event of a multiples pregnancy, CFC surrogates receive  $10,000 in additional compensation for each fetus. Your contract will outline timing and any related details based on medical circumstances.

Do intended parents cover legal fees?

Yes. Journey  legal fees are covered. You should not be asked to come out of pocket for any expenses.

Is compensation the same in every state?

That depends on the agency. CFC pays first-time surrogates $60,000 except in DC and MD, where surrogates are paid $65,000.
